Description

The Care Manager LMFT will function as a core member of a multidisciplinary Oncology care team through a structured Care Management LMFT program in which outreach, education, care coordination and periodic follow up are provided. Acts as an advocate and helps patient gain access to resources while confronting their personal issues related to cancer diagnoses and survivorship. The position takes responsibility for management of a therapeutic caseload (individual therapy to adults) providing timely charting and record keeping. The Care Manager LMFT may provide evidence-based treatments or work with other mental health providers when such treatment is indicated.

Required Qualifications:

• Master's Degree Marriage and Family Therapy or Behavioral Science.

• Bachelor's Degree.

• Upon hire: California Clinical Social Work Associate License. Or

• Upon hire: California Marriage and Family Therapist License. Or

• Upon hire: California Clinical Social Worker License. Or

• Upon hire: California Marriage and Family Therapist License. Or

• Upon hire: California Marriage and Family Therapist Intern License. Or

• Upon hire: California Psychologist License.

• 2 years In the field or in a related area.

Preferred Qualifications:

• 2 years Ambulatory clinic or hospital.
